Patient Clinical Summary: PROMIS reporting capabilities were improved and expanded.
Designer/Flexible Reports: The PROMIS team improved the patient navigation, selection and grouping criteria and expanded flexibility of parameter selection to generate various reports.
Electronic Charting:
Modules were designed and developed to enable electronic charting and clinic visits management.
- Integrated Care in Interior Health: In partnership with Interior Health (IH) information technology services, the PROMIS team developed an electronic chart for chronic disease management. The chart is based on the workflow processes of the Penticton Integrated Health Centre, which cares for individuals with kidney disease, heart disease and diabetes. The chart is fully interfaced with the IH Meditech system and aligned with Canada Health Infoway and provincial Electronic Health Record strategies.
- Independent Hemodialysis Program: Building on the work done with IH, the PROMIS team developed and implemented an electronic chart for the cross-province independent hemodialysis program.
The long term vision is to develop electronic charting components for all renal programs, gradually phasing out paper charting.
Train-the-trainer Initiative: A unique train-the-trainer program for PROMIS proved successful in Interior Health with over 85 people trained and data entry increased by 30%.
Vascular Access: A comprehensive set of modules was designed and developed to track creation and monitoring of vascular access.
Comorbidities: The modules used for assessing patient comorbidities and related events were redesigned. Responsiveness to user requirements and increased training has led to a steady improvement in data entry and quality. Currently, 89% of patients have a comorbidity assessment in PROMIS, which enables the renal community to more clearly and reliably define outcomes.
Medication Reconciliation: Through refinements to the PROMIS database, BCPRA is supporting medication reconciliation across the continuum of renal care – from clinic to hospital admission/discharge and back to the clinic. Medication reconciliation report tools, as well as a tutorial, are now available through PROMIS.
Data management improvements: A set of reports and procedures to regularly monitor data quality were developed.
User Training/Support: We have increased the hours of our PROMIS help desk, developed a rapid response strategy that delineates urgent versus non urgent requests, and are developing an expanded training program, including a new training manual.
