Alberta Health Services’ Advance care planning welcome page provides links to background information on the regional development of ACP including a link to their 2008 ACP Logic Model framework. It also offers a section for healthcare provider resources including clinician tools and access to their on-line learning modules.
Canadian Hospice Palliative Care web site offers several resource documents for allied healthcare professionals (under Professionals).
Canadian Institute for Health Information (CIHI) recently released a report titled "Health Care Use at the End of Life in Western Canada" and "Health Care Use at the End of Life in British Columbia".
Canadian Institute of Health Research (CIHR) Cross Cultural Palliative Care NET published Andrea Con's Cross-Cultural Considerations in Promoting Advance Care Planning in Canada in February 2008.
