The BC renal care community has developed a comprehensive end-of-life (EOL) framework to support the delivery of high quality care for people with kidney disease in the last years, months, or days of their lives, regardless of where they live in BC.
The framework report, titled “
END-OF-LIFE FRAMEWORK - Recommendations for a Provincial End of Life Care Strategy” is aligned with the BCPRA strategic priority of creating quality outcomes and better value for patients.
Key principles of the framework include close collaboration with palliative care colleagues/programs in the geographic health authorities, as well as alignment with the Ministry of Health Services palliative care strategy.
As outlined in the framework report there are four distinct aspects needed for full renal EOL program development being:
- patient identification,
- symptom assessment & management,
- the advanced care planning process,
- care in the final stages, including grief and bereavement
To help support a well planned, longitudinal EOL program and development across the BC renal programs a champions training workshop was hosted by the BCPRA in May, 2010. Presentation and handouts from the EOL Champion Training Workshop have been posted.
In addition to the framework, this resource section with web links was developed to support EOL education and guideline development. This list includes recent articles, books, reports and presentations on EOL care as well as links to Health Authority Renal EOL resources and external educational programs.
